We believe in the power of the patient-caregiver relationship and are looking for professionals who share our passion for delivering outstanding, compassionate care.<\/p>\n<p>If you are a dedicated professional committed to helping clients maintain their independence at home with dignity, we encourage you to apply.<\/p>\n<hr \/>\n<h3><b>What You&#8217;ll Do: Key Responsibilities<\/b><\/h3>\n<p>Under the direct supervision of an RN Case Manager, you will provide hands-on personal care and support based on a customized patient care plan. Your duties will include:<\/p>\n<ul>\n<li><b>Provide Personal Care:<\/b> Assist clients with bathing, personal hygiene, grooming, dressing, and other activities of daily living (ADLs).<\/li>\n<li><b>Assist with Mobility:<\/b> Safely transfer, position, and ambulate clients using proper body mechanics and equipment such as Hoyer lifts, slide boards, and sit-to-stand aids.<\/li>\n<li><b>Execute Therapeutic Activities:<\/b> Assist with prescribed range-of-motion exercises, application of non-sterile dressings, and use of assistive devices.<\/li>\n<li><b>Perform Specialized Care:<\/b> As directed by the RN, provide catheter care (including bag changes), colostomy care, and assistance with established bowel programs.<\/li>\n<li><b>Support Nutrition &amp; Household Needs:<\/b> Plan and prepare nutritious meals according to dietary plans, assist with feeding, perform light housekeeping, and shop for groceries to maintain a clean and healthy home environment.<\/li>\n<li><b>Collaborate and Document:<\/b> Accurately document all care provided, report any changes in the client&#8217;s condition immediately to the RN Case Manager, and maintain clear communication with the care team.<\/li>\n<li><b>Maintain Professionalism &amp; Safety:<\/b> Adhere to all agency policies, including the Exposure Control Plan, and maintain a professional, supportive relationship with all clients, including those with complex medical or psychological needs.<\/li>\n<\/ul>\n<hr \/>\n<h3><b>What You&#8217;ll Bring: Qualifications &amp; Skills<\/b><\/h3>\n<p><b>Required:<\/b><\/p>\n<ul>\n<li>Current Certified Direct Care Professional (CDCP) certification <b>OR<\/b> a minimum of two (2) years of professional direct care experience.<\/li>\n<li>A valid driver&#8217;s license, reliable insured vehicle, and willingness to travel to client homes.<\/li>\n<li>Ability to perform the physical duties of the role, including lifting, transferring, and repositioning clients (with or without reasonable accommodation).<\/li>\n<li>Must pass a pre-employment physical and drug screening.<\/li>\n<li>Demonstrated reliability, punctuality, and a strong sense of responsibility.<\/li>\n<li>A positive, team-oriented attitude with the ability to follow care plans and take direction from RN Case Managers.<\/li>\n<\/ul>\n<p><b>Preferred:<\/b><\/p>\n<ul>\n<li>Previous experience in a home health care setting.<\/li>\n<li>Experience caring for clients with complex conditions such as dementia, ALS (<span class=\"math-inline\"><span class=\"katex\"><span class=\"katex-html\" aria-hidden=\"true\"><span class=\"base\"><span class=\"mord mathnormal\">A<\/span><span class=\"mord mathnormal\">m<\/span><span class=\"mord mathnormal\">yo<\/span><span class=\"mord mathnormal\">t<\/span><span class=\"mord mathnormal\">ro<\/span><span class=\"mord mathnormal\">p<\/span><span class=\"mord mathnormal\">hi<\/span><span class=\"mord mathnormal\">c<\/span><\/span><\/span><\/span><\/span> <span class=\"math-inline\"><span class=\"katex\"><span class=\"katex-html\" aria-hidden=\"true\"><span class=\"base\"><span class=\"mord mathnormal\">L<\/span><span class=\"mord mathnormal\">a<\/span><span class=\"mord mathnormal\">t<\/span><span class=\"mord mathnormal\">er<\/span><span class=\"mord mathnormal\">a<\/span><span class=\"mord mathnormal\">l<\/span><\/span><\/span><\/span><\/span> <span class=\"math-inline\"><span class=\"katex\"><span class=\"katex-html\" aria-hidden=\"true\"><span class=\"base\"><span class=\"mord mathnormal\">S<\/span><span class=\"mord mathnormal\">c<\/span><span class=\"mord mathnormal\">l<\/span><span class=\"mord mathnormal\">eros<\/span><span class=\"mord mathnormal\">i<\/span><span class=\"mord mathnormal\">s<\/span><\/span><\/span><\/span><\/span>), MS (<span class=\"math-inline\"><span class=\"katex\"><span class=\"katex-html\" aria-hidden=\"true\"><span class=\"base\"><span class=\"mord mathnormal\">M<\/span><span class=\"mord mathnormal\">u<\/span><span class=\"mord mathnormal\">lt<\/span><span class=\"mord mathnormal\">i<\/span><span class=\"mord mathnormal\">pl<\/span><span class=\"mord mathnormal\">e<\/span><\/span><\/span><\/span><\/span> <span class=\"math-inline\"><span class=\"katex\"><span class=\"katex-html\" aria-hidden=\"true\"><span class=\"base\"><span class=\"mord mathnormal\">S<\/span><span class=\"mord mathnormal\">c<\/span><span class=\"mord mathnormal\">l<\/span><span class=\"mord mathnormal\">eros<\/span><span class=\"mord mathnormal\">i<\/span><span class=\"mord mathnormal\">s<\/span><\/span><\/span><\/span><\/span>), or quadriplegia.<\/li>\n<li>Proficiency in advanced care skills, including Hoyer lift transfers, catheter care, bowel programs, and range of motion exercises.<\/li>\n<\/ul>\n<hr \/>\n<h3><b>Why Join PHCS?
