Pressure ulcers are localized injuries to the skin and underlying tissue, usually over a bony prominence. They result from prolonged pressure on the skin, often in combination with shearing forces, friction, and moisture. Classifying Pressure Ulcers: Understanding the Stages
Pressure ulcers are classified into stages based on the depth of tissue damage:
- Stage 1: Non-blanchable erythema of intact skin. The area may be painful, firm, soft, warmer, or cooler compared to adjacent tissue.

- Stage 2: Partial-thickness skin loss involving the epidermis, dermis, or both. The ulcer presents as a shallow open ulcer with a red-pink wound bed, without slough. It may also present as an intact or ruptured serum-filled blister.

- Stage 3: Full-thickness tissue loss. Subcutaneous fat may be visible, but bone, tendon, or muscle are not exposed. Slough may be present but does not obscure the depth of tissue loss. May include undermining and tunneling.

- Stage 4: Full-thickness tissue loss with exposed bone, tendon, or muscle. Slough or eschar may be present. Often includes undermining and tunneling.

- Unstageable: Full-thickness tissue loss in which the base of the ulcer is covered by slough (yellow, tan, gray, green, or brown) and/or eschar (tan, brown, or black) in the wound bed. Until enough slough and/or eschar is removed to expose the base of the wound, the true depth, and therefore stage, cannot be determined.

- Deep Tissue Pressure Injury (DTPI): Intact or non-intact skin with localized area of persistent non-blanchable deep red, maroon, purple discoloration or epidermal separation revealing a dark wound bed or blood-filled blister. Pain and temperature change often precede visible tissue damage. This injury results from intense and/or prolonged pressure and shear forces at the bone-muscle interface.
