PHCS is looking to add an additional Physical Therapist to our team. We are looking for a PT that would like to do home health visits part-time.  The schedule is very flexible.  You will be able to develop your own schedule.  You can adjust your schedule however you see fit, as long as the weekly visit frequency is in accordance with the Physical Therapy Plan of Care and the individual client’s needs. The position would require you to do Physical Therapy visits in our clients’ homes in the Metro Milwaukee area. The visits may require evaluating and providing physical therapy services to home health clients with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders. This includes client and caregiver education; discharge planning; and consultation to other professionals, clinics or agencies. Pay is per PT visit with an additional premium for assessments.

POSITION QUALIFICATIONS:

As a Home Care Physical Therapist, you must be dependable, professional, and energetic with a positive and empathetic personality. You will also need to have strong organizational, assessment, and problem-solving abilities. It is also important that you display excellent communication and interpersonal skills as well as the ability to interact well with patients, families, physicians, and staff.

Specific qualifications for the Home Care OT position include:

  • Positive, Can-Do Attitude! (this is very important)
  • Highly organized (able to keep your schedule and organize your visit route, complete all required documentation in a timely manner, collaborate with other members of the care team both within and outside the organization.)
  • Valid, current, and unrestricted PT license in the state of Wisconsin
  • 1-3 years of Physical Therapy in home care is a plus, but not required
  • Strong sense of initiative and urgency in addressing unexpected patient crises
  • Demonstrated knowledge regarding exposure to body fluids and chemicals, as well as to some hazardous equipment
  • Current TB skin test (within 90 days of hire) or willing to take a TB test upon hire
  • Ability to pass a pre-employment drug screen and physical
  • Ability to travel as required. Current valid driver’s license and automobile insurance.
  • Possess excellent computer skills (must be capable of charting electronically and adapting to technological changes)
  • Physical abilities to adequately perform described duties (no physical restrictions)

The employee must occasionally lift and/or move up to 50 pounds. The employee must occasionally reposition or help ambulate patients. Specific vision abilities required by this job include close vision, distance vision, peripheral vision and depth perception.
